We’re seeking a dedicated Hospice Registered Nurse (RN) Case Manager to join our team for a full-time position (40 hours per week).

Pay and Schedule

$40–$46/hour

  • Monday–Friday, 8:00am–5:00pm
  • On-call and weekend rotation approximately every 5 weeks
  • Includes weekend coverage (Saturday and Sunday)
  • Includes holiday participation (minimum one summer and one winter holiday)
  • $6/hour hold rate plus time-and-a-half for visits during call

Responsibilities

  • Manage hospice patient caseload across home and facility settings
  • Perform patient assessments and develop individualized care plans
  • Provide skilled nursing care and symptom management
  • Coordinate with interdisciplinary team members
  • Maintain documentation using Homecare Homebase
  • Participate in on-call rotation

Territory

  • Primary coverage area includes Berks County
    • May be asked to assist with Montgomery, Chester, and Bucks Counties
  • Patients are assigned geographically to support efficiency
  • Typical day includes approximately one hour of drive time
  • Mix of facility and home-based hospice care

Requirements

  • Active RN license
  • At least one year of clinical experience (acute care, home health, or hospice preferred)
  • Valid driver’s license and reliable transportation
