We’re seeking a dedicated Hospice Registered Nurse (RN) Case Manager to join our team for a full-time position (40 hours per week).
Pay and Schedule
$40–$50/hour
- Monday–Friday, 8:00am–4:30pm
- On-call and weekend rotation approximately every 5 weeks
- Includes weekend coverage (Saturday and Sunday)
- Includes holiday participation (minimum one summer and one winter holiday)
- $6/hour hold rate plus time-and-a-half for visits during call

Responsibilities
- Manage hospice patient caseload across home and facility settings
- Perform patient assessments and develop individualized care plans
- Provide skilled nursing care and symptom management
- Coordinate with interdisciplinary team members
- Maintain documentation using Homecare Homebase
- Participate in on-call rotation
Territory
- Coverage includes Reading, Wyomissing, Sinking Spring, and Berks counties
- Patients are assigned geographically to support efficiency
- Typical day includes approximately one hour of drive time
- Mix of facility and home-based hospice care

Requirements
- Active RN license
- At least one year of clinical experience (acute care, home health, or hospice preferred)
- Valid driver’s license and reliable transportation
