Job Summary

We are seeking a compassionate and dedicated Social Worker to join our team.  As a Social Worker, you will play a vital role in providing psychosocial support and resources to our patients and their families. You will work closely with our interdisciplinary team to ensure that our patients receive the best possible care.

Core Responsibilities

• Assess the psychosocial needs of patients and families
• Provide counseling and crisis intervention
• Develop and implement care plans
• Educate patients and families on community resources
• Assist with funeral arrangements and financial planning
• Maintain clinical records
•  Participate in interdisciplinary team meetings

Qualifications

• We’re excited to train social workers that are passionate about hospice care!
• Bachelor’s degree in social work from an accredited institution (master’s degree in social work is preferred)
• One year of experience in a healthcare setting preferred
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
•  Valid driver’s license and reliable transportation
• Active Wisconsin Social Work license 

Work Environment

• Mix of facility and home-based care
• Monday – Friday, 8am-5pm (some on-call and weekends)
• Centralized territory assignment
