About the Role
We are seeking an experienced Physical Therapist (PT) to join a leading home health provider serving Montgomery County, MD. In this role, you’ll provide therapy services within 1–2 assisted living facilities (ALFs) — helping minimize travel while still enjoying the autonomy of home health.
As a home health PT, you’ll be a key member of a multi-disciplinary care team, delivering one-on-one rehabilitative care and supporting patients’ quality of life.
Responsibilities
- Provide physical therapy visits to patients within designated assisted living facilities.
- Perform evaluations including mobility, range of motion, strength, endurance, balance, gait, transfer status, and safety assessments.
- Develop and implement individualized care plans with interventions such as therapeutic exercises, gait training, and balance activities.
- Collaborate with nursing and the broader care team on multidisciplinary care planning.
- Educate patients and families on rehabilitation techniques, safety, and independence.
- Document care delivery and patient progress using Homecare Homebase (HCHB) EMR system on a tablet.
Qualifications
- Current Maryland Physical Therapist (PT) license.
- Graduation from an accredited Physical Therapy program (APTA approved).
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Proficiency with EMR systems; Homecare Homebase (HCHB) experience preferred.
Compensation & Productivity
- $84,500 – $145,600 annually (based on experience and productivity)
- Weekly pay + direct deposit
- Productivity standard: 25 points per week
- SOC (Start of Care) = 2.5 points
- Evaluation = 1.25 points
- Recertification = 1 point
- ROC Recertification = 2 points
- Routine Visit = 1 point
- Discharge Visit = 1 point
- Mileage reimbursement at the IRS rate
