Physical Therapist (Home Health) – Southeast Metro Area, CO

$3,500 Sign-On Bonus | $80K–$100K Salary | No Weekends or On-Call Required

Key Responsibilities

  • Deliver in-home physical therapy services in accordance with physician orders
  • Conduct evaluations, develop care plans, and complete OASIS assessments
  • Provide therapeutic interventions: gait training, mobility, range of motion, etc.
  • Educate patients and families on treatment plans, safety, and adaptive equipment
  • Collaborate with interdisciplinary care team and assist with discharge planning
  • Supervise Physical Therapy Assistants (PTAs) and Certified Nursing Assistants (CNAs)

What You’ll Need

  • Active Physical Therapist (PT) license in Colorado
  • Minimum of 1 year of PT experience (home health preferred; SNF/hospital/rehab accepted)
  • Current CPR certification
  • Valid driver’s license and auto insurance
