About the Role
As a Palliative Nurse Practitioner at our organization, you’ll provide compassionate care to patients with serious illnesses and their families. You’ll be a key member of our palliative team, working in both outpatient and inpatient settings.
Key Responsibilities
- Assess patients’ physical, psychological, emotional, spiritual, and social needs
- Prescribe medications and treatments to manage symptoms
- Document patient visits and discussions using our EMR system (Netsmart)
- Communicate findings and recommendations to the care team
- Participate in discharge planning to ensure safe transitions across the care continuum
- Stay current on goals of care conversations, Advance Health Care Directives, and DMOST
- Contribute to staff and community education on palliative care and end-of-life issues
Qualifications
- Master’s degree and certification as a Certified Registered Nurse Practitioner (CRNP)
- Current Delaware nursing license
- CPR certification
- Minimum of two years of nursing experience, preferably in hospice or palliative care
- Certification (or willingness to pursue) as an Advanced Nurse Practitioner in Hospice and Palliative Care
Physical Requirements
- Ability to bend, squat, lift, and turn patients
- Capable of lifting 30 to 60 lbs, following industry safety standards

Schedule and Compensation
- Monday – Friday, 8 am – 4:30 pm with some on-call responsibilities
- $100,000 – $112,000 base compensation
- Mileage reimbursement at the IRS rate
Work Location and Coverage Area
- Home-based visits in New Castle County
