About the Role

As a Palliative Nurse Practitioner at our organization, you’ll provide compassionate care to patients with serious illnesses and their families. You’ll be a key member of our palliative team, working in both outpatient and inpatient settings.

Key Responsibilities

  • Assess patients’ physical, psychological, emotional, spiritual, and social needs
  • Prescribe medications and treatments to manage symptoms
  • Document patient visits and discussions using our EMR system (Netsmart)
  • Communicate findings and recommendations to the care team
  • Participate in discharge planning to ensure safe transitions across the care continuum
  • Stay current on goals of care conversations, Advance Health Care Directives, and DMOST
  • Contribute to staff and community education on palliative care and end-of-life issues

Qualifications

  • Master’s degree and certification as a Certified Registered Nurse Practitioner (CRNP)
  • Current Delaware nursing license
  • CPR certification
  • Minimum of two years of nursing experience, preferably in hospice or palliative care
  • Certification (or willingness to pursue) as an Advanced Nurse Practitioner in Hospice and Palliative Care

Physical Requirements

  • Ability to bend, squat, lift, and turn patients
  • Capable of lifting 30 to 60 lbs, following industry safety standards

Schedule and Compensation

  • Monday – Friday, 8 am – 4:30 pm with some on-call responsibilities
  • $100,000 – $112,000 base compensation
  • Mileage reimbursement at the IRS rate

Work Location and Coverage Area

  • Home-based visits in New Castle County

Productivity Expectations

  • 20 visits per week
