About the Role

We’re seeking a compassionate and experienced Hospice Registered Nurse (RN) to support patients and families through one of life’s most meaningful transitions. As a member of our interdisciplinary team, you’ll deliver holistic care in the home setting—managing symptoms, offering emotional support, and ensuring dignity at every stage.

This field-based role offers autonomy, flexibility, and deep personal connection. You’ll manage a caseload of 10–15 patients, averaging 4–5 visits per day, with time built in for meaningful patient interaction and documentation.

Pay & Benefits

  • $37–$45/hour, based on experience
  • Mileage reimbursement at IRS rate

Key Responsibilities

  • Perform initial and ongoing assessments in the patient’s home
  • Develop and adjust individualized hospice care plans
  • Provide pain and symptom management aligned with best practices
  • Offer patient and family education, emotional support, and end-of-life guidance
  • Coordinate with physicians, chaplains, social workers, and aides
  • Accurately document care using KanTime EMR
  • Attend IDG meetings and participate in continuous quality improvement

Qualifications

  • Active RN license in the state of New Hampshire
  • Minimum 1 year of hospice or home health experience ideally
  • Excellent clinical judgment, emotional intelligence, and documentation skills
  • Valid driver’s license and reliable transportation
  • Familiarity with KanTime EMR preferred (training provided
