Hospice Registered Nurse (RN) Case Manager

This caring hospice organization recognizes the significance of each precious, end-of-life journey.  We will do all we can to provide an empowering hospice experience to each patient we have the privilege of working with.

We are seeking a talented Hospice Registered Nurse for 40 hours per week.

Job Summary:

As the RN case manager, you will plan, organize, and direct hospice care for your patients. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. You will be the central point for the patients care plan and will help lead and mentor our Hospice Aides. Our case managers will provide care through our service area.

Qualifications:

  • A passion for hospice care and the ability to build relationships with both patients and their family members
  • Current licensure and CPR certification
  • The ability to work efficiently with little direct supervision
  • A valid driver’s license and an insured automobile that is in good working order
  • Acute care experience in an institutional setting or home care experience is preferred, but not required. We’re excited to train new RN’s that are passionate about hospice care
  • Graduation from an accredited school of nursing
  • Must be comfortable treating COVID patients

Schedule:

  • Monday – Friday; 8a-5p with some on-call and weekend rotation

Work Location:

  • Primarily facility based (65%)
  • Home based hospice care (35%)

Coverage Area:

  • Will be assigned a territory 

Productivity:

  • Managing an average caseload of 12-15 patients

EMR Used:

  • Homecare Homebase

Compensation: 

  • $67,000 – $85,000 per/year
  • On-Call and Call-Back compensated
  • Mileage reimbursement at $0.58 per/mile

  • Android smart phone
  • Ample PPE
  • Necessary supplies
