Hospice Registered Nurse (RN) Case Manager

They are seeking a talented Hospice Registered Nurse (RN) for 40 hours per week.

POSITION SUMMARY:

Assesses, prescribes, delegates, coordinates, and evaluates the nursing care provided. Ensures provision of quality care for selected groups of patients through utilization of nursing process, established standards of care, policies and procedures. He/She is responsible for the written Nursing Care Plan, acts as the patient advocate, and is responsible for the direction and supervision of any personnel assigned to him/her. The RN is responsible to the Unit Nurse Manager or House Supervisor.

QUALIFICATIONS:

  • Home Health or Hospice experience (highly preferred)
  • 2 years of med/surg or equivelent nursing experience if no previous Home Health or Hospice experience (required)
  • Must be flexible and family oriented (required)
  • Current RN license in the State of Oregon (preferred)Graduate of an accredited school of professional nursing (required)
  • Valid Driver’s License, reliable transportation and current automobile and liability insurance (required)
  • Tech savvy (preferred)
  • Strong documentation, communication, and good assessment skills (required)
  • Current BLS (required)

Job Type:

  • Full-time

Hours per week:

  • 40

Schedule:

  • Monday -Friday; 8a-4:30p; some call rotation
  • 1 weekend every 4 weeks (every 5-6 weeks when fully staffed)

Compensation:

  • $37.50 – $63.80 Per/Hour (compensation based on years of experience)
  • Mileage reimbursement at IRS rate
  • $30 monthly cell phone allowance

Work Location:

  • Out in the field providing patient visits (home)

Coverage Area:

  • 60-mile radius of the Hermiston Office (includes Pendleton)

Productivity Requirements:

  • 4-5 patient visits per day 
  • Managing an average caseload of 10 patients
