A mission-driven and growing hospice provider is seeking a compassionate, experienced Hospice Registered Nurse (RN) Case Manager to join its team. This is a full-time, field-based role offering flexible scheduling, strong leadership support, and the opportunity to make a lasting impact on patients and families during their end-of-life journey.

Position Highlights:

  • Branch Census: ~30 patients
  • Caseload / Average Visits: Caseload of approximately 12 patients; daily visits vary but typically average 4–5 per day. Several partner facilities have multiple patients.
  • Leadership: Works closely with the Executive Director and Director of Clinical Services
  • Schedule: Monday–Friday daytime schedule with an on-call rotation. Rotates with RN, Executive Director, and Director of Clinical Services; includes some weeknights and at least one weekend per month.
  • Territory: Preference for candidates living in the southeast area and able to focus on patients in the Greenwood/Franklin area. Occasional coverage may be required in other parts of Indianapolis (including the northeast side such as Carmel, Fishers, and McCordsville). On-call coverage may include any service area.
  • EMR: Netsmart

Key Responsibilities:

  • Conduct physical and psychosocial assessments and develop individualized care plans
  • Manage pain and symptoms using evidence-based interventions
  • Provide education and emotional support to patients and families
  • Collaborate with interdisciplinary team members for coordinated care
  • Accurately document all assessments, care plans, and follow-up in the EHR (Netsmart)
  • Participate in on-call rotation and quality improvement efforts

Qualifications:

  • Active RN license
  • Hospice or home health experience preferred but not required
  • Open to candidates with 2+ years of Med-Surg or critical care experience; prior exposure to end-of-life care or home-based settings is strongly valued
  • Strong assessment, documentation, and communication skills
  • Committed to providing compassionate care—this is more than just a 9–5
  • Team-oriented and flexible with territory coverage as needed
