We are partnering with a growing hospice organization serving patients throughout the Greater Philadelphia region. This organization is known for delivering compassionate, patient-centered care through interdisciplinary teams in both home and facility settings while maintaining a supportive, team-oriented clinical environment.
We’re seeking a dedicated Hospice Registered Nurse (RN) Case Manager to join our team for a full-time position (40 hours per week).
Pay and Schedule
$85,000–$105,000 annually based on experience
- Monday–Friday schedule w/ some on-call rotation
- Flexible scheduling structure with leadership support

Responsibilities
- Manage hospice patient caseload across home and facility settings
- Perform patient assessments and develop individualized care plans
- Provide skilled nursing care and symptom management
- Coordinate with interdisciplinary team members
- Maintain accurate and timely clinical documentation
- Maintain documentation using WellSky EMR
- Support patients and families throughout the hospice journey
Territory
- Coverage includes Philadelphia, Bucks, Chester, Delaware, and Montgomery counties
- Ideal candidate location is Doylestown, PA
- Patients are assigned geographically to support efficiency
- Mix of facility and home-based hospice care
- Travel throughout the territory is required

Requirements
- Active RN license
- At least one year of clinical experience (acute care, home health, or hospice preferred)
- Valid driver’s license and reliable transportation
