They are seeking a talented Home Health Registered Nurse for 40 hours per week.

POSITION SUMMARY:

Asses, delegate, coordinate, and evaluate the nursing care provided. Ensures provision of quality care for selected groups of patients through utilization of nursing process, established standards of care, policies and procedures. She/he initiates the Plan of Care and necessary revisions and is responsible for the overall care of the patient. She/he initiates appropriate measures to prevent disease exacerbation, promote safety, makes referrals to rehabilitative services and coordinates all services provided by other disciplines/organizations. . The Home Health RN is directly responsible to the Assistant RN Manager of Home Health and or Director of Home Health and Hospice and her designee. Responsible for carrying out the mission, vision and quality commitment of the agency. 

QUALIFICATIONS:

  • Home Health experience (highly preferred)
  • 1-2 years of med/surg OR ICU nursing experience if no previous Home Health experience (required)
  • Must be flexible and family oriented (required)
  • Current RN license in the State of Oregon (preferred)Graduate of an accredited school of professional nursing (required)
  • Valid Driver’s License, reliable transportation and current automobile and liability insurance (required)
  • Tech savvy (preferred)
  • Strong documentation, communication, and good assessment skills (required)
  • Current BLS (required)

Job Type:

  • Full-time

Hours per week:

  • 40

Schedule:

  • Monday -Friday; 8a-4:30p; NO CALL
  • 1 weekend every 3-4 weeks (every 5-6 weeks when fully staffed)

Compensation:

  • $37.50 – $63.80 Per/Hour (compensation based on years of experience)
  • Mileage reimbursement at IRS rate
  • $30 monthly cell phone allowance

Work Location:

  • Out in the field providing patient visits (home)

Coverage Area:

  • 30-mile radius of the Pendleton Office
  • May be asked to help cover Hermiston Office and surrounding area when needed

Productivity Requirements:

  • 6-7 points per day (weighted by visit type & drive time)
    • Example: a start of care = 3 points
  • Managing a caseload of 18-22 patients
