About the Role

We’re seeking a dedicated Home Health Registered Nurse (RN) to deliver skilled, compassionate care to patients in their homes. You’ll manage a caseload of 15–20 patients, typically completing 4–6 visits per day. This is a meaningful opportunity to work independently while being part of a team that values trust, flexibility, and clinical excellence.

We welcome candidates with at least one year of Medicare-certified home health experience but will also consider strong critical care nurses (ICU, ED, or step-down) with the right mindset and initiative.

Pay & Benefits

  • $37–$45/hour based on experience
  • Mileage reimbursement at IRS rate ($0.67/mile)

Key Responsibilities

  • Perform in-home assessments and develop care plans tailored to each patient
  • Deliver skilled nursing visits and monitor clinical outcomes
  • Educate patients and families on medications, disease processes, and safety
  • Collaborate with physicians, therapists, and other care team members
  • Document visits in KanTime EMR
  • Participate in case conferences and quality improvement initiatives

Qualifications

  • Active RN license in New Hampshire
  • 1+ year of Medicare-certified home health or recent critical care experience
  • Excellent clinical, documentation, and communication skills
  • Reliable transportation and valid driver’s license
  • Familiarity with KanTime EMR preferred (training available)
