Responsibilities:

  • Provide skilled nursing care under the direction and supervision of a Registered Nurse and physician.
  • Assist in implementing individualized care plans and documenting patient progress and outcomes.
  • Monitor patient conditions and report changes to the RN Case Manager and interdisciplinary team.
  • Educate patients and caregivers on medications, treatments, and disease management.
  • Perform skilled nursing procedures using standard homecare equipment and supplies.
  • Maintain accurate and timely clinical documentation and reports.
  • Communicate effectively with physicians, healthcare teams, patients, and families.
  • Participate in regular case conferences and care coordination activities.
  • Promote patient safety, comfort, and independence in the home setting.

Qualifications:

  • Passionate about providing exceptional home healthcare.
  • 1-2 years of recent LPN experience preferred (Home Health, Med-Surg, Long-Term Care, Rehab, or Skilled Nursing).
  • Active LPN license in Maryland.
  • Current CPR certification.Experience with wound care, medication management, and patient education preferred.
  • Strong organizational and communication skills.
  • Basic computer proficiency (Homecare Homebase EMR experience a plus).
  • Valid driver’s license, reliable transportation, and an acceptable driving record.

Schedule:

  • Monday – Friday; 8:00am-4:30pm (No On-Call!) with occasional weekend rotation

Compensation:

  • $65,286 per year
  • Mileage reimbursement at the IRS rate

Work Location:

  • Out in the field providing patient visits (home)

Coverage Area:

  • Upper Prince George’s County – grouped by zip codes, primarily covering College Park/Hyattsville

Productivity Requirement:

  • 27 points per week
