Your RoleWe’re seeking a compassionate Physical Therapist to deliver personalized, one-on-one care in patients’ homes throughout the scenic Lake Winnipesaukee region. This role offers the autonomy to develop and implement treatment plans while being supported by a collaborative team. Key Responsibilities

• Conduct comprehensive evaluations to develop tailored treatment plans.
• Provide therapeutic care, mobility training, and adaptive equipment recommendations.
• Educate patients and families to support recovery and independence.
• Collaborate with a multidisciplinary team to optimize patient outcomes.
• Maintain accurate documentation using KanTime EMR.

What We’re Looking For

• Required:
• Bachelor’s degree or higher in Physical Therapy.
• Active New Hampshire PT license.
• Strong communication and problem-solving skills.
• Proficiency with EMR systems.
• Preferred:
• Home health experience (training available for motivated candidates!).
